Enrollment Information
What to expect:
Once you have all the documentation gathered, head over to either Baker Center (Punta Gorda) or Pumpkin Patch (Port Charlotte) and ask for a Family Advocate. An available Family Advocate will assist you with your application, ask a couple additional questions regarding your household, and answer any questions you may have.
Bring the following to complete your application:
Acceptable Proof of Birth Documents:
- Birth Certificate: Original or certified copy
- Passport: US or foreign passport
Commonly accepted:
- Car registration
- FPL bill
- Water Bill
- Voters registration
- Mortgage or rental agreement
We can NOT accept:
- Bank statements
- Driver's License
- Health insurance documents
- Junk Mail
- Mail pieces that are not listed above
This includes:
- 12 months worth of paystubs
- 2024 1040 tax form
- W2
- Child support
- VA benefit
- Current TANF/SSI/SSD/ SNAP statement (if you receive SNAP, that is ALL the documentation needed for proof of income!)
***Income documentation only needed if applying for free full-day services***
- Current physical with height, weight, hearing, vision, and blood pressure (if applicable)
- Up-to-date Immunizations
- Hemoglobin and Lead results
- Dental screening or exam documentation (not needed at the time of registration) will need when enrolling
